Accepted Insurance Plans

Mount Sinai has network agreements with many health insurance companies.

Keep all scheduled appointments and procedures with Mount Sinai, and avoid delaying routine health screenings, procedures, or important tests.

If you receive insurance through your employer, tell your Human Resources or Benefits Department that you want to keep in-network access to Mount Sinai Health System’s high-quality providers and services.

Call Anthem at the number on the back of your insurance card and let them know you want to keep in-network access to the Mount Sinai caregivers and services you know and trust.

You have the opportunity to change to a health plan Mount Sinai participates with during the Marketplace Open Enrollment period, which begins Saturday, November 1, and continues through Thursday, January 15.

To change plans, visit the NY State of Health website during the Open Enrollment period.

If you enrolled in your Marketplace plan through NY State of Health please call 1-855-355-5777 (TTY: 1-800-662-1220). Calls are answered Monday through Friday from 8 am to 8 pm, and on Saturday from 9 am to 1 pm.

If you enrolled in your Medicaid plan through the NY State of Health website, please call 1-855-355-5777 (TTY: 1-800-662-1220). Calls are answered Monday through Friday from 8 am to 8 pm, and on Saturday from 9 am to 1 pm.

If you have Medicaid through your cash assistance case, you can change your plan by calling New York Medicaid Choice at 1-800-505-5678 (TTY 888-329-1541). Calls are answered Monday through Friday from 8:30 am to 8 pm, and on Saturday from 10 am to 6 pm. Have your Medicaid number handy when you call.

Important Notes

Plan changes made before the 15th of the month will be effective the first of the following month. Plan changes made after the 15th of the month will be effective the first of the second following month. This means that if you choose to change your plan for December 1, for example, you must do this before November 15.

If you are switching to another plan, please check with the health insurance company to make sure that your doctors and other providers are in their network.

To continue to receive covered care at Mount Sinai with in-network benefits, you may be able to change plans to a plan that Mount Sinai participates with. You can go to New York State’s NYS Provider & Health Plan Look-Up website for help locating a Child Health Plus plan that participates with Mount Sinai and is available in your area.

To continue receiving care at Mount Sinai with in-network benefits, you can change your Child Health Plus plan at any time during the year by calling NY State of Health at 1-855-355-5777 (TTY: 1-800-662-1220). Calls are answered Monday through Friday from 8 am to 8 pm, and on Saturday from 9 am to 1 pm.
